If you want to enrol more than one child to the program, you have to fill out this form for each child.

Terms and conditions: 1. Testing and conditioning times 1.1 Set session times will be confirmed prior to the commencement of the first session, and this time schedule will be confirmed with the parent and/or coach via email. 1.2 Please ensure that your child is on time for every session. 1.3 Should you wish to discuss a matter with one of the trainers please make an appointment outside of the session times. 1.4 A late collection fee of R30.00 will be added to your invoice should you collect your child after the designated time. Please support us and arrive on time. 2. Holidays There will be no testing or conditioning sessions on weekends or public holidays. During school holidays there will be no formal testing and conditioning sessions taking place. Personalised holiday programmes will be provided to each child during the last formal session before the school holidays commence. 3. Fees (applicable to the party responsable) 3.1 Fees are payable in advance every calendar month. 3.2 The first monthly instalment is due upon completion of this contract of enrolment. 3.3 Fees are then due before the 3rd day of each calendar month or 7 days from invoice date. 3.4 Interest of 10 % will be charged to your account for overdue payments for that particular month. 3.5 Normal fees are payable for school holidays and there will be no credit for holidays or any absenteeism what-so-ever. 3.6 Arrear fees will be handed over to the attorneys, and all attorney fees will be for your cost. If any claim against the Applicant (you as responsible party) is placed by the COMPANY (STACKS (PTY) LTD) in the hands of its attorneys, and whether or not action is instituted, and without prejudice to any other rights which the COMPANY (STACKS (PTY) LTD) may have the COMPANY (STACKS (PTY) LTD) shall be entitled to recover all legal costs incurred by it, including, without departing from the generality of the a foregoing, all attorney/client charges, tracing fees and such collection commission as the COMPANY (STACKS (PTY) LTD) is obliged to pay its attorneys, from the Applicant (you as responsible party). 3.7 No cash payment will be accepted. Notice Should you wish to withdraw your child from the program for whatever reason, STACKS will require a calendar month notice. Notice must be given in writing and the parent/guardian shall remain liable for all fees payable during the notice period. 5. Reregistration An annual non-refundable registration fee of R 150.00 is charged for new applications. If you re-register 1 month prior to contract end no re-registration fee will be charged. When failing to re-register before this specified period a new registration will need to be done and so the normal registration fee will be charged. 6. Medication 6.1 Medication must be clearly labelled with the child’s name / dosage / frequency and SHOULD BE ON THE CHILD AT ALL TIMES. 6.2 The child is required to inform the trainers at each session of all medicine required/ that might be required during the session. The trainers must also be informed of the location of the medicine and it must be easily accessible to the trainers. 6.3 With sever conditions/allergies the medicine must be put in the medicine box, which will be provided. The box will be kept out of reach of the child/ren at all times. It is the responsibility of the parent to take the medicine when the child/ren is collected. 7. Health policy 7.1 Parents are requested to make alternative arrangements for children who exhibit any of the following symptoms: Temperature/fever, diarrhoea, undiagnosed skin rash, eye infection, vomiting, tonsillitis and other infectious diseases. 7.2 Children who are brought to testing or conditioning sessions and are exhibiting obvious symptoms (like the above- mentioned symptoms) will not be allowed to partake in any activity without a completed doctors letter stating that they can take part in physical activity. 7.3 Please inform us if your child/ren will be absent from any session, especially if absenteeism will be for more than 2 consecutive sessions. 7.4 In case of an extreme emergency, paramedics are contacted first, and parents are telephoned directly thereafter. 8. Unforeseen and dangerous circumstances 8.1 STACKS reserve the right to cancel and/or halt any training and/or testing sessions with the Applicant should this fulfilment be delayed or rendered impossible by dangerous weather conditions (including but not limited to high winds, hail, excessive precipitation, wildfires, thunderstorms, downbursts, tornadoes, waterspouts and tropical cyclones) 8.2 STACKS reserve the right to cancel and/or halt any training and/or testing sessions with the Applicant should this fulfilment be delayed or rendered impossible by war, invasion, insurrection, riot, order of any government, municipal or civil authorities, breakdown, accidents, labour disputes or any other cause beyond the reasonable control of STACKS. 8.3 There will be no credit, refunds for, or rescheduling of these cancelled sessions as the safety of all the children is STACKS’s main concern. If any training session and/or testing session is deemed unsafe by the STACKS personal, whatever the cause might be, and no practical alternative arrangement can be made at that point in time, the session will be cancel and/or halt effective immediately. 9. Clothing 9.1 Child/ren are required to wear comfortable exercise attire during all STACKS sessions. 10. Email address/ Telephone number details Inform us immediately of any change of email address / contact numbers – these are essential during emergencies and all future communication for example: invoices, statements, reports, newsletters etc. will be via e-mail. 11. General Information 11.1 We encourage your child/ren not to miss any sessions, as each session is beneficial to your chid/ren and is structured in such a way that each session builds on the skills learnt during the last session. Exception when child is sick. 11.2 Children that have not been registered and have not paid the sign-up fee with STACKS will unfortunately not be will not be allowed to participate in the sessions. 11.3 One bottle of sun block (according to the child’s skin type) and a hat is compulsory for each child. 11.4 Please timeously return any STACKS items accidently found in your child’s possession. Our equipment is essential to our work and is costly to replace. 11.5 Please inform us if your child will be on holiday during the school terms. 11.6 Any queries or problems no matter how insignificant they are deemed, must be reported to the STACKS directors directly. PLEASE READ THE FOLLOWING EXERCISE ADVISE AND EXPLAIN TO YOUR CHILD CAREFULLY • Follow the instructions given to you by the STACKS trainers very carefully. • Ask the trainers to help you when you are struggling or when you don’t understand what to do. • Work at your own pace as you feel comfortable and concentrate on learning the correct techniques. • If you feel any pain, dizziness or discomfort at any time during the session, inform your trainer immediately. • If your child suffers any injury, illness or condition in the future, please tell us and complete in another form. • For your child to benefit optimally from the program we recommend practising the skills and techniques learnt at home.

Informed Consent MUST BE SIGNED BY PARENT/LEGAL GUARDIAN Please refer to the pre-exercise questionnaire and complete. I understand and accept all policies and procedures of STACKS as written above and agree to accept the terms and conditions contained herein. The information provided in the pre-exercise questionnaire regarding my child’s health is, to the best of my knowledge, correct. I agree to inform you immediately if there are any changes to the information provided above whilst my child is participating in the STACKS (PTY) LTD Training Program. I understand my child participates entirely at his/her own risk and must exercise due care to ensure his/her personal health and safety, and that of others. I understand that STACKS (PTY) LTD cannot be held liable for any injuries or losses that might occur during the testing and/or conditioning sessions. I hereby fully indemnify STACKS (PTY) LTD and its personnel, the organizers, all sponsors and partners, volunteer groups, medical personnel and all other parties from any direct or indirect loss or damage however caused arising from my child’s participation in the testing and/or conditioning sessions or related to these sessions. I hereby indemnify, defend and hold harmless the venue, its owner, its management company, its respective affiliates and all of its respective officers, directors, partners, members, and employees from and against all demands, suits, judgments, settlements, claims, damages to persons and/ or property, fines, liens, losses and other liabilities, including reasonable attorneys’ fees arising out of or in any way related to the testing and/or conditioning sessions, including claims for loss or damage to any property, or for death or injury to any individual.” I have explained to my child that they need to listen and follow any directions or advice affecting their safety and that of others, given to me by STACKS (PTY) LTD. I hereby authorize the trainers of STACKS (PTY) LTD “in loco parentis” of the child in the event of any medical emergency. Such authorization shall include without limitation the right to procure the services of a medical practitioner or hospitalization and to sign any documentation necessary for the child to receive medical treatment, which may be considered reasonably necessary. The parent/guardian shall reimburse STACKS (PTY) LTD for any costs incurred in the exercise of such authorization. My child is in good health, physically fit and sufficiently trained to participate in and we understand the risks associated with these testing and conditioning sessions. Herewith I undertake to pay the STACKS (PTY) LTD fees on time and understand that legal steps without notice will be taken against me in case of failure to pay. I give permission and herewith apply for admittance for the above-mentioned child to commence in the above selected program option provided by STACKS (PTY) LTD. I, the undersigned, contractually agree to the following terms and conditions of enrolment.
